Looks to me like maybe acid or some other chemical did this. Look at his hands. Look at how smooth the edges of the wound, excuse me, Hole are. The scull looks as if it has been worn down some. Smooothed out if you will. There appears to be some sort of residue. I don't know, I'm going by what it looks like to me.
