Escharotomy: Surgical Release of Full-Thickness Burns to Chest and Arms
An escharotomy is a surgical operation used in patients with severe burns. Full-thickness burns create eschar, a hard, leather-like layer of dead tissue. During the first 48 hours after a major burn, damaged capillaries leak fluid and fluid resuscitation contributes additional fluid, causing swelling beneath this unyielding tissue. Since eschar does not expand, pressure rises within the enclosed compartment. In a limb, this can cut off blood flow to the hand or foot and lead to loss of the limb. Over the chest or trunk, it can prevent the ribcage from expanding, leaving the patient unable to breathe. To relieve this pressure, a surgeon divides the eschar to the underlying fat with a scalpel or cutting diathermy, allowing the tissue to separate. On the chest, the usual arrangement consists of two cuts along the anterior axillary lines, connected across the upper chest and beneath the ribs, forming a pattern sometimes called the Roman breastplate. The case is described as involving a 35-year-old man burned in a house fire, with the trunk and upper limbs most severely affected, matching the procedure's two textbook indications. The patient is alive and receiving active treatment. Escharotomy is performed to preserve a life or limb, not after death.  · |
