Can't tell for sure but it looks like a separation of the head from the vertebra. Wearing helmets and being in a five point hitch makes this more likely to happen.

I had a look thru some shots of Ayrton and not one of them showed him wearing that sort of clothing. None of them had "sen..." on them. Maybe it was a new design but not sure if it's him in the pic.
