My guess is that there was no ignition, but rather a case of too high pressure and/or faulty hose/parts.

A high pressure explosion can easily result in hardware parts being blown into a body.

Also, under very high pressure, a hose can whip around with great force. Certainly sufficient to cut a body wide open.

judging by the paint and the combo cylinder carrier, id say he made the mistake of uncapping a pressurized tank for repainting without doing safety measures.
