At the fracture site itself, the most important immediate complication is blood loss. This may be considerable, followed by damage to the femoral artery and vein and to the common peroneal and medial popliteal nerves. There will also be muscle damage with the danger of gas gangrene as well as osteomyelitis. Inspection distally shows early gangrene of the toes and swelling of the ankle.
