The question mark is pointing to nephrons of a kidney. They are normally on the inside, so we rarely see them. There are about one million in each kidney- that's right, a million in each kidney, so they are very small. That is one of the reasons high blood pressure is so dangerous. As the pressure gets too high it damages the nephrons, and if the sufferer would want to keep living, he/she would be required to undergo multiple weekly dialysis sessions, each of which lasts hours, because the dialyzer then has to function as a kidney, because destroyed nephrons cannot.
