I'm thinking their psychiatric issues, and more specifically their capacity for hallucination is key. They may have believed that they had to sever their own heads to kill an evil spirit or something like that. Or they wanted to decapitate themselves to make "the voices in their head" stop. This is obviously extreme delusion and desperation. Very sad.
