The toilet is not an unusual place for women to give birth when they are out of reach of medical support and, especially, when they are experiencing a lot of blood loss. It is easy to imagine how this unfortunate mother may have successfully delivered her baby, but then had complications before delivering the placenta which would cause her to hemmorage. (Delivery of the placenta enables the uterus to contract and is essential to control bleeding after childbirth.) While sitting on the toilet, she likely passed out from sudden blood loss while delivering the placenta and never regained consciousness. Then neglected, her wee baby would naturally die too.
