Very interesting cause as a safety feature the doors of an elevator are not suppose to open unless the elevator is in place and there are sensors to detect this maybe at the time this was the case and the girl took it for granted and walked in without looking and fell to her death. I still always look before I get inside.

Yeah, there had to be many complete failures to safety systems. If an elevator does that, adequate inspection was not done or not properly done. Good point.
