On April 10, 2010, a catastrophic air disaster occurred near Smolensk, Russia, involving a Tupolev Tu-154 aircraft operating as Polish Air Force Flight 101. The tragedy resulted in the deaths of all 96 people on board, including the President of Poland, Lech Kaczyński, and his wife, Maria, other senior Polish military officers, government officials, and relatives of the Katyn massacre victims. The flight was en route from Warsaw to Smolensk to attend the 70th-anniversary ceremonies of the Katyn massacre, a mass murder of Polish intellectuals, politicians, and military officers by the Soviets during World War II.

The aircraft attempted to land at Smolensk North Airport, a former military airbase, in extremely poor visibility due to thick fog. The airport lacked an ICAO-compliant instrument landing system, typically used in most developed countries, and relied on a non-directional beacon for non-precision approaches. The Polish Tu-154, modified to use a Western-style ILS, descended below the normal approach path, struck trees, rolled, inverted, and crashed in a wooded area near the runway.

Investigations by Russian and Polish authorities found no technical faults with the aircraft but concluded that the crew failed to conduct the approach safely under the given weather conditions. Serious deficiencies were identified in the organization and training of the Air Force unit involved, leading to several high-ranking resignations within the Polish military. Notably, various conspiracy theories have circulated, alleging deliberate sabotage, which were later dismissed by official investigations

The first final accident report, created by MAK, was published on 12 January 2011, and placed the majority of the blame for the accident on the pilots for descending too low without being able to see the ground.

I'm sure every agency that investigates plane crashes could use that very general statement.
