Some of these people are run in to the ground with the amount of hours they work. His reaction time /judgement was probably affected by tiredness.

It never ceases to amaze me how many people dive headfirst into stupid situations like that. People put their heads in machines that malfunction, people look down barrels of rifles that misfire, there's always someone willing to put their face somewhere it shouldn't be.
