Too much load, too high and too little lift truck. Plus, why are all those pedestrians walking around? The driver did remember his H&S training and went straight into “oh what am I like” mode and held his head in both hands, so that’s alright. |

The man was operating the straddle stacker truck incorrectly, from what I can see. A turn should never be made with the load raised any higher than about 12 inches from the ground as this destabilises the truck. He is trying to save time by turning with the load up like that. And that looks like a pallet of drinks which are heavy, usually between 600 and 900 kilograms (1300-2000lbs). Even using a heavier truck requires care when driving these pallets. However, one could argue that his boss is to blame for this accident since most normal warehouses would be using reach-trucks for this task as they are more stable, and can lift more weight. They are also more expensive and require more operator training. Using straddle trucks like this gives me the idea that the warehouse is not being run in a professional manner. We would not allow a situation like this to unfold at our company's warehouses in the UK or the Netherlands. |

I worked in warehouses for years. The weight of the load was obviously way above that lift's rated capacity, especially at that height. The operator is absolutely at fault. If your boss tells you to do something dangerous like that, the answer is "no", especially if there are people around; because the one time things go wrong will be the one time you kill someone. He probably did that same maneuver a hundred times before. He'll never operate a lift again. Someone has to tell that man's family he was killed... needlessly. What a fucking tragedy, easily preventable.

It's not the operator's fault that some jackass was mindlessly standing around and chatting with his buddy, while in the path of this undeniably highly skillfull and perfectly executed "warp-speed decent" maneuver. Get out of the way, JACKASS! "When there is a heavy load above you, pay attention." |
