The tasers failed due to the lack of spread of the probes. Either could have touched the taser to his ankle or tight to get the better spread for the full stun effect.

Preventable, yes.

I should have watched her cam before posting.
Her commands were good.
She recognized he was EDP and should have requested a five man deployment before approaching.

Both were too close for cartridge deployment, should have went for a cartridge stun then used the TASER for the second point of contact to disable him. This was covered in the TASER update back in 2016.

After the TASER deployment she backed up and got tangled in her wires. She got some pain from the stun. Instead of complaining about it, she should have stayed focused on the threat. (Also a good reason why every TASER user should be tased during training. You can deal with the pain and keep going.)

But after the deployment, she can't get her gun out of her holster, then points it squarely at her partner the entire time she was up. She manages to not laser everything but her partner. Thankfully she didn't have any sympathetic fire or her partner would have taken rounds in the vest and probably one in the neck or back of the head.

So I should have watched her whole video, so I am editing my post. I stopped after the shooting. Didn't know she hit herself in the arm, I thought she just got tangled up in the wires.

TASERs and pistols are two handed weapons to keep you from shooting yourself. The TASER probe is a small fish hook, a #8 I believe. To remove, you pinch the skin around the probe and jerk it out in one quick motion. The barbed end might be 2mm in width, if that. Every TASER user is taught removal and shown how small the business end of the probe is. For comparison, the probe end might be a #20 gauge syringe size, if that. Her breakdown is unfortunate because had she been alone, she could be seriously hurt by the suspect or dead.

What I assume is a relative that called 911 is saying, 'I told you not to shoot him!' after the shooting. I understand that reaction from him after witnessing all that, but as the investigation showed, it was justified. Unfortunate that he had to witness it.

Of note, you can see one of his rounds penetrated at least thru his chest, possibly exiting his chest.
No autopsy report, but I would hazard a guess that at least one of those rounds was instantly fatal and destroyed his heart, but he still had oxygenated blood to his brain and body that allowed him to keep moving forward (moving at all).

It is entirely understandable when a family member says it, they are emotionally invested in the person.
