Inhaling requires muscle/diaphragm contraction. Exhaling is a passive process. Shooting himself in the head cancelled the nerve impulse to the diaphragm, allowing it to relax. The groan you hear is the rushing air coming out of his lungs past his vocal cords. The lungs are now empty and the diaphragm is relaxed.

When you die, your muscles relax and anything that you may or may not have been holding in will come out. So air will leave the lungs. Sometimes if a body is moved and there's still air in the lungs, the body can moan or groan.
