I can't imagine what it must feel like to have a knife digging and scraping around the vicinity of the spinal cord. Almost without fail, the victim appears agitated as the knife reaches that point, and their whole body spasms when that connection is severed. It must be one of the most painful experiences a person can feel, even if its only for a split second.

I have several fused vertebrae in my lower spine after decades of back and leg pain. Prior to the fusion surgery, I had to have a test performed on my spine to locate the source of the highest level of pain. This test was called a "discogram". They lay you on an special exam table face down with your lower back curved in such a way to make it easier for the doctor to insert the needles into your discs.

I knew I was in for a ride when one of the assistants handed me a rubber ball to squeeze during the procedure and began gently rubbibng my shoulder in a soothing, motherly manner.

I'm not exactly how, but the test is designed to actually induce pain. It's basically equivalent to smashing your fingers with a hammer and asking "does this hurt". It was the most painful thing I've felt, ever. This was over ten years ago now and I still get an unsettling feeling when I think about it.

I always wonder how long they are "alive" after their head is separated. You would think it must be for a few seconds or more. The second guy's heart is still beating, not that it has any affect on his head, but I guess technically his body is still alive.

And, based on the many other videos like this on DR, this executioner is a pro with a sharp knife. I'd hire him.

As far as him being conscious, not very long. Shock and blood loss would put him out cold within a few seconds of the knife slitting his throat.
After that, I'd say his heart would cease beating with the lack of blood circulating.
