Not much info on this; but obviously he was struck with a grinding wheel that came apart.

Yeouch, just missed the jugular ! Being in the auto body repair business for nearly 30 years I'm surprised not to have seen more accidents like this, glad I haven't. |

This is why you should always inspect your equipment prior to operation, that should be common sense with a thin rock that rotates at 1800rpm. I could use this picture for safety training at work.
