They're speaking spanish. They're all speaking at the same time. He seems disoriented but he's repeating something three times that seems to be: "There's something in the very back" (and that's when they pull the flap of skin). And then the Dr. Asks" "pero está consciente?", which means: "but are you fully conscious?"
