Typically when someone that is fat in the stomach has a surgery, this can be a result of a secondary infection in which they must keep the wound open and clean often so it heals very slowly. You would rinse the wound many times daily and eat high protein diets.
This, basically. I am an RN and I see wounds like that all the time. Interestingly enough I have never had one patient--fat, skinny, or just right--complain that the wound care, which generally involves packing, hurts at all. In fact many patients can do it themselves once they are discharged. If everything goes well they actually heal very quickly.
