It's lymphodema. The blood supply is still good (the skin in the shot from above is bright pink) so it's not diabetes complications or anything like that. They've also obviously had compression stockings on but haven't changed them (there is a definite dividing line between the pink raw skin and the thicker skin above) and are probably in for whatever complications arose from that. My guess is unchecked lymphodema in a wheelchair-bound patient, which would explain why the lymphodema is severe, seems localised to the calf and foot and the feet themselves seem completely dysfunctional.
