The kid undoubtedly knows what animals he may encounter outside (especially ones that can be found in the burrows of the bush rats he was hunting), and what they look like.

If he didn't see it after it bit him, the medical staff could have identified it by the set of symptoms he exhibited and how severe the reaction was.

considering there were lab tests and surgical procedures done, medications given, and antivenin acquired, i think it's preeetty safe to assume that he was cared for at a legit hospital by staff with medical training
