Neonatal breast enlargement, also called neonatal mastauxe, is swelling of the breast tissue in newborns caused by maternal hormonal influence. Falling maternal estrogen after birth is thought to trigger prolactin production in the baby, stimulating the breast. This neonatal effect is not early puberty. Some degree of enlargement occurs in about 70% of newborns, and milk secretion occurs in 5 to 20%.

Uncomplicated enlargement is managed with observation and reassurance. Infection can cause mastitis or a breast abscess, which may require drainage. Squeezing or repeatedly emptying the breast can prolong milk secretion and is believed to introduce infection, although one study of 106 infants found prolonged secretion without any infections.

Premature thelarche

Breast development may occur in girls aged <3 years, and can then spontaneously regress. This is often seen in girls under the age of 3 years and is caused by maternal oestrogens in the early months. There is fairly static breast development before true puberty eventually occurs at the normal time. It is a benign condition confirmed by:

Absence of any other signs of puberty.
Normal growth with appropriate bone age (ie no growth spurt).
Minimal increase in breast tissue with time (can even decrease).
Appropriate uterine dimensions for age (ultrasound) with normal endometrial echo and no vaginal bleeding.

I've seen precocious puberty in maybe, 5 or 6 year-olds, mainly because their fathers are using some sort of testosterone creams and lotions, and when they touch their children enough with the stuff it triggers early development, but 18 DAYS OLD????????????????? I talk with parents and caregivers when I pick up my oldest great niece at school. She is 10 now.
