Man Has Myiasis in His Leg

Myiasis is the infection of a fly larva (maggot) in human tissue. This occurs in tropical and subtropical areas. Myiasis is rarely acquired in the United States; people typically get the infection when they travel to tropical areas in Africa and South America. People traveling with untreated and open wounds are more at risk for getting myiasis. Fly larvae need to be surgically removed by a medical professional.

Considering that's not America, safe to say it wasn't a black widow or brown recluse/violin spider..
This was a beast of far greater proportions responsible.

Check out the maggots... (loaded with em'... If you didn't notice them, look a bit closer..moving all about in there..)

That entire hole is/was necrotic. (I say "Was" necrotic due to the maggots eating all of the dead tissue away.)

Lucky if this person still has this limb.
At least in a usable manner anyway.

There are LOTS of maggot species that eat LIVE flesh, rather than dead flesh.

Many species cause these infections and wounds, and are definitely NOT cleaning them up.
