Lag Bolt Through HAND
This is the result of horseplay with an AMPACT tool by someone on another crew at the powerline contractor where I work.

AMPACT tools are engaged by firing a special powder loaded cartridge within the tool which reduces the time and effort required to tap a power line.

And of course - this idiot had mary jane in his system, lost his job, and may still lose the hand....
