It looks like a right hand that was holding something (like his dick), which was itself enclosed by some sort of rigid sticks ...one on either side. It was a 'body modification' using a gun. I think at this point he needs to do some 'touch up' work with a razor blade or scalpel perhaps. |

In addition, he had applied a rather severe tourniquet at the wrist, either prior to or after using the gun. This would account for the pale white color of the hand which is difficult to understand at first.
