He doesn't look dead to me. The rope isn't cutting into his neck much, and there is no pooling. I'm thinking this is sexual. If that's the case, he'll most likely go to far and end up here for real soon enough.
The body colouration indicates death. He no doubts choked and the soiling is a common thing when people die.
