Yah, that's a hard read. The trauma to the arm is one of the things that makes it so hard. If it was just the head I'd even guess a gunshot wound from a high powered rifle. I suppose it still could be a shot to the arm and head. Could be shrapnel from a bomb perhaps? I'd guess the head injury came first from the posturing of the hand.
