I think this really happened. I don't think it was as depicted in the picture, though.

The real story went, as I heard it, is a zoo worker gave an elephant an enema to get it going (it had been constipated for several days), when the elephant let go with a sudden rush.

The force of the evacuation shoved the man back against the wall of the elephant's pen hard enough to knock him out. As he lay unconscious, he was buried in the elephant's excrement and was suffocated by it.

If it is a true story, erf... quite a nasty way to go.
