It was a cascade of failures. The limousine was described as a junker; regulations about limo inspection were incredibly lax at the time. The driver had significant amounts of THC in his blood. The crash occurred when the brakes failed at a T intersection at the bottom of a steep hill, the sole of the driver's shoe was worn through from desperately trying to pump the brake. Thanks, I read through it. It's hard to believe the State Police managed to keep the NTSB investigators from doing ANY investigation until they were through with their criminal investigation. Normally the Federal Government Investigators are absolutely in charge from the start, and everyone else comes AFTER them. This is the first time I ever heard of such a thing.
