It actually appears, to me, to be real. His slow death is due to the nature of the ligature: he's using a bathroom towel, which would tend to spread his weight over a larger area of his neck than would a rope. The same principle explains why a tank can move over softer ground than can a wheeled vehicle, as the treads spread the weight of the tank over a larger area of ground than do tires (although the ultra-crappy M4 Sherman had treads so narrow it may as well have been a wheeled vehicle).
