Very fake. Look at the caliber of the pistol compared to what was produced by the "exit wound". Second, there is no brain matter at all - just "blood". Had they gone with a much smaller squib, it would have been more realistic.

This is indeed fake. The girl is b actress named Pamela Sutch. The scene is from Snuff Perversions 2. It's a collection of random scenes directed by different directors made to look like a complied snuff movie bought in an alley somewhere dark. It's a pretty entertaining watch. Better than the shit August Undergound put out.
