Bodyparts just don't fall of, you die from bloodpoisoning from the gangrene, far before that would happen. My biggest quess is that he mutilated himself.
You should read up on mountaineering, digits fall off all the time , in extreme conditions , gangrene setts in long before septisemia( Blood poisoning) !
