The source of entry was obviously external and maggots aren't parasites. Nobody gets them from eating sushi (tape worms yes, but not maggots). Most likely some minor wound on her scalp was allowed to get infected and fester, and she just let it go until you see these results.

These maggots were found in a woman's brain after repeated complaining of severe headaches. It was found that she had severe brain cancer and was infested with maggots.

Guy had some form of cancer but never sought treatment because he felt no pain...well yeah, if half your brain's eaten away your pain centers might get destroyed in the process!
