That's the heart the doc's holding. Just below the heart is the deflated lung. You can see he was probably a smoker or lived in an area of poor air quality---note the black dots.

I am pretty certain that is neither the heart nor the lung because there's no ribs involved. That looks like pectoral muscle and surrounding soft tissue.

I think the pectoralis major is much more upward! but you made a good point concerning the ribs...hmmm...
