Adam Air Flight 574 Cockpit Recording
The aircraft was reported to have had a long history of problems with its inertial reference system (IRS), which measures various flight parameters. During the flight, the pilots encountered issues with the IRS, leading to discrepancies in the plane's position as indicated by the flight management system displays. The pilots were attempting to troubleshoot the malfunctioning navigation system when the aircraft deviated from its course and entered a severe thunderstorm. As the situation in the cockpit escalated, the aircraft rolled steeply to the right and entered a dive, accelerating beyond its maximum speed. This resulted in the plane disintegrating in midair. The black boxes, recovered from the ocean floor after six months of delays, revealed the final moments of the flight. The investigation pointed to a combination of equipment malfunction, inadequate pilot training, and poor maintenance practices as contributing factors to the crash.
