NewCal Aviation de Havilland Canada DHC-4 Caribou Crashes in Test Flight
On August 27, 1992, an aviation accident occurred involving a de Havilland Canada DHC-4T Caribou operated by NewCal Aviation. The aircraft, registered as N400NC, was undertaking a test flight when it crashed shortly after takeoff from Gimli Airport, MB, at around 10:20 AM. Tragically, all three crew members on board were killed in the accident. The aircraft was completely destroyed. The Transportation Safety Board (TSB) of Canada conducted an investigation into the crash. It concluded that the incident was caused by the failure to fully disengage the control gust lock system before flight. This critical oversight led to the re-engagement of one or more locking pins after the aircraft had taken off, resulting in the crash. |

TSB concluded that the control gust lock system had not been fully disengaged prior to flight and that one or more of the locking pins had become re-engaged after lift-off. Nuff said. |
