Hot air balloon falling from the sky

Due to fabric failure in the top of the balloon, this pilot was killed as hundreds of fellow balloonist and spectators watched in horror.
Some how the pilot had time to radio the crew to get spectators out of his way as he decended knowing about what was going to happen to him.
As a result of this accident there have been many improvements in the annual inspection process of the fabric in these aircraft.
